Telehealth Consent
This consent notice explains important limitations of remote care through affiliated clinical partners.
Remote care
Telehealth care may be provided remotely through online intake, messaging, video or audio tools, digital records, and care coordination. Patients must provide accurate and complete health information.
Clinical review
Clinicians may determine that telehealth is not appropriate for a concern and may direct a patient to labs, identity verification, pharmacy review, follow-up, in-person care, or another resource. Treatment is not guaranteed, and prescriptions are not guaranteed.
Benefits, risks, and limitations
Telehealth can improve access and convenience, but it has limitations. Technical failures may occur, remote review may not capture every clinical detail, and some symptoms require hands-on examination or urgent care.
Questions and emergencies
Care-related questions should be directed through the support or clinical partner channel when available: Clinical partner contact method is pending final partner configuration.
